Turkish Republic Identity Card containing the Turkish Republic Identity Number (if under 18, accompanied by a guardian)
Health All medical records, if any, of the person for whom the report will be issued.

For Guardianship Report: A cover letter accompanied by the Turkish Ministry of Justice or law enforcement.

For Age Determination: A cover letter accompanied by the Turkish Ministry of Justice or law enforcement (with the guardian or legal representative).

Note: Fees are charged according to the type of health report issued.
